    * By submitting this form to videoevening@gmail.com, you are declaring that you have read the terms and conditions included on the conference's video evening web page, and agree to them. This includes the declaration that you are the owner of the copyright of the submitted material or that you have the permission of the copyright owner to make the submission

    * The closing date for submission of this form is the 8th of September in order to allow sufficient time for shipping the material by the video submission deadline of the 8th of October. Please ensure that you ship your video timeously.
